Your first visit
What to bring
- Photo ID
- Your dental insurance card
- A list of the medicines you take
- Any recent dental X-rays
Questions about insurance? Call us and we'll gladly check your dental benefits.
Before you come in
- Call (888) 202-8586 to schedule your visit.
- If dental visits make you nervous, tell us when you call. We can talk about sedation options.
- If you have X-rays from another dentist, you can ask that office to send them to us, or bring a copy.
- Give yourself a few extra minutes to find Suite 225 and check in.
For your privacy, please do not send health or medical details by email. We will go over your health history with you directly.
